Vitamin D may be the single most over-promised vitamin of the past decade. It has been credited with preventing everything from cancer to depression, debunked as a miracle in trial after trial, and then quietly re-recommended for narrower, more sensible reasons. For women over sixty, the picture is genuinely worth understanding, because there are a few things vitamin D does that matter, and there are several things it does not do that get bundled in by enthusiastic marketing.

Vitamin D is, slightly oddly, more of a hormone than a vitamin. The body synthesises it in the skin when ultraviolet B (UVB) light from the sun strikes a cholesterol precursor; the liver and kidneys then convert it into the active form. This active form has two well-established roles:
- It enables the gut to absorb calcium efficiently. Without enough vitamin D, dietary calcium is poorly absorbed regardless of how much is in the diet.

Why Women Over 60 Are At Higher Risk of Low Levels
A few specific factors converge in our age group:
- Skin synthesis declines with age. A seventy-year-old produces roughly half as much vitamin D from the same amount of sunlight as a twenty-year-old.
- More indoor time. Working life and many retirements involve less sun exposure than school years did.
- Sensible sun protection. SPF 30 reduces vitamin D synthesis substantially. This is a real trade-off, not a problem; skin cancer risk is far more serious in our age group, and a small daily supplement is the better solution than reducing sun protection.
- Latitude. From late October through early March in most of the UK and northern Europe, the sun is too low in the sky to drive much vitamin D synthesis at all.
- Some medications and conditions. Long-term steroid use, certain anti-seizure drugs, gastric bypass surgery, and some inflammatory bowel conditions reduce absorption or alter metabolism.
The combination is why many public-health bodies now make a routine recommendation for adults over the age of about 65 to take a small daily vitamin D supplement, particularly in winter months.
How Much Vitamin D Do We Actually Need?
Guidance varies internationally, but the broad consensus for adults over 60 is around 800 to 1,000 IU (20 to 25 micrograms) of vitamin D per day, taken from food, supplement, or a combination.
- The NHS guidance on vitamin D recommends 10 micrograms (400 IU) daily for adults, with everyone in the UK considered for a supplement during autumn and winter, and adults over 65 considered for one year-round.

Doses above 4,000 IU daily are not generally recommended without medical supervision; very high doses can cause hypercalcaemia (too much calcium in the blood) and have been associated with adverse effects in some trials.
What the Recent Research Has and Has Not Shown
The last decade has been a busy time in vitamin D research, and the picture has become clearer in some areas and less clear in others.
Where the evidence is strong:
- Correcting genuine deficiency (blood levels below roughly 25 nmol/L) reduces fracture risk and improves muscle function in older adults.
- Combined calcium and vitamin D supplementation reduces fracture risk in institutionalised older adults, though the effect in community-dwelling older adults is smaller and more variable.
Where the evidence is mixed:
- General supplementation in adults with already-adequate levels does not appear to confer significant additional benefit for fractures, falls, cancer, or cardiovascular disease in the largest recent trials, including the VITAL trial summarised in PubMed.
- High-dose intermittent supplementation (large doses every few weeks) has produced disappointing or even slightly worse outcomes in some studies and is no longer generally recommended.
Where the evidence is genuinely uncertain:
- Effects of vitamin D on mood, cognition, immune function, and respiratory infection rates are inconsistent across trials.

How Do You Know If You Are Low?
A simple blood test (25-hydroxyvitamin D, or 25(OH)D) is the standard. Common interpretive thresholds, in nmol/L:
- Below 25: deficiency.
- 25 to 50: insufficiency.
- 50 to 75: adequate for most people.
- 75 to 125: optimal in the view of most clinical bodies.
- Above 250: supplementation should usually be reduced.
A blood test is not always necessary. For most healthy older women without specific risk factors, a daily 800 to 1,000 IU supplement during autumn and winter (and year-round for those over 65 in northern latitudes) is a sensible approach without testing. For women with osteoporosis, recurrent falls, malabsorption conditions, or unusual dietary patterns, testing once is worth doing.
The symptoms of low vitamin D are mostly vague (general tiredness, mild muscle weakness, low mood) and overlap with many other conditions, so symptom-based diagnosis alone is unreliable.
Food Sources Worth Knowing
Vitamin D from food is helpful but, for most older women in the UK or northern Europe, insufficient on its own. Useful sources:
- Oily fish: salmon, mackerel, sardines, herring.
- Egg yolks (a small contribution per egg).
- Mushrooms exposed to UV light during cultivation.
- Fortified foods: many breakfast cereals, some milks (especially plant milks), some yoghurts.
A portion of salmon (about 100 g) provides roughly 400 to 600 IU. Two portions of oily fish per week is a long-standing nutritional recommendation in the UK.
A Practical Pattern
The approach I share with most clients, in plain language:
- Take a daily 800 to 1,000 IU vitamin D3 supplement, particularly between October and March in northern latitudes; year-round if over 65.
- Eat oily fish twice a week if you enjoy it, or once a week as a minimum.
- Get short, sensible periods of sun exposure on the face and hands when the weather allows in summer (5 to 15 minutes around midday, less if your skin is fair).
- If you have osteoporosis, recurrent falls, or unexplained muscle weakness, ask your GP for a 25(OH)D blood test.
- Avoid mega-dose supplementation unless prescribed.

Vitamin D and Other Supplements
A quick honest tour of common companions:
- Calcium. Worth pairing in older women with osteoporosis or low dietary calcium. Spread the intake across meals; single very high doses do not absorb as well and have been linked to small cardiovascular concerns in some studies.
- Vitamin K2. Plausible biological role in directing calcium to bones rather than soft tissues; current trial evidence is interesting but not conclusive.

As ever, more is not better. A handful of well-chosen daily habits beat a drawer full of supplements, almost every time.
